| $20Get your student athletes ready for the upcoming school year! Memorial Regional Health is hosting Sports Physical Night—a convenient, one-stop event for middle and high school students who need their annual sports physicals.
Cost: $20 (cash or check only)
Open to all middle and high school athletes
No appointment needed—walk-ins welcome!
This is a great opportunity to knock out a required physical, support local athletics, and keep your kids on the field and in the game.
Complete your portion of the physical form and bring it along with glasses/contacts, if you wear them. Physical form can be found at moffatsd.org under the K-12 Athletics tab, or picked up at MRH front desk.
Rapid Care will be CLOSED at 5:00 p.m. to everything but physicals.
